Wellness Visit Nurse Practitioner.
Nurse Practitioners (NP) needed to conduct In-Home Health Assessments in the greater Ventura/Central Coast and surrounding areas. (Many locations) We offer year-round opportunities: Full-time or Part-Time.

Our healthcare providers conduct comprehensive, in-home, Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides. Within the visit, the clinician reinforces the importance of staying current on covered preventive services while identifying potential gaps in care, which stem from unnecessary hospitalizations and ER visits.
